Applications should be made as early as possible to avoid disappointment!
In any case please apply at least 4 weeks before each course begins.
To create a hard copy of this form:
Either:
from the "Edit" menu, click "Select All", then from the "File" menu, click "Print" and follow the instructions.
Or:
from the "Edit" menu, click "Select All", click "Copy" then "Paste" into your word processor package.
Print the form off, complete it and post it to:

- please enclose a cheque for the course fee
- make the cheque payable to "SYCSA"
- please enclose a SAE.

E-mail the completed form as an attachment (up to Word 2000 format) to courses@sycsa.org.uk
SYCSA will confirm by e-mail.
Once you have received confirmation:
- to secure your place,
- you should then post a cheque,
- made payable to "SYCSA",
- with a copy of the confirmation,
- to the address above.

Personal injury insurance SYCSA has third party insurance liability for the boats. You may wish to take out your own personal insurance to cover injury to yourself. Fitness declaration.

- To the best of my knowledge, I do not suffer from any medical condition that will automatically disqualify me from taking part in sailing.
- To the best of my knowledge I do not suffer from Epilepsy, Giddy spells, Asthma, Diabetes, Angina or other heart conditions, Brittle bones and I am fit to take part in these sessions
Membership of SYCSA for under 18 year olds
By completing this form, you agree to become a Member of SYCSA.
You must get your parents or guardians agreement.
- Due to the Young Persons Safety Act 1995, a young person under the age of 18 must become a Member of SYCSA.
- There is no membership fee.
- I apply to be a Member of SYCSA
- I understand that as a Member, I am entitled to participate in the activities of SYCSA, and must comply with the rules and regulations of SYCSA.
